How do I register for Outpatient Radiology Services?

Patients arriving for outpatient services are to report to the Logan Regional Medical Center's main Admission/Registration desk to allow for the introduction of their personal and billing information into the facility health information system. This process can take between 30 minutes and one hour to complete, and it is best to allow additional time if you have a scheduled appointment. A photo identification (driver's license) and insurance information is required to complete the registration process. All patients are fitted with a patient identification armband (bracelet), and serves as a means to verify the identity of the patient while testing is being conducted.

The Radiology Department of Logan Regional Medical Center also provides services to physician's offices through patient referrals. A written referral must be presented at the time of registration to allow the Radiologic Technologist to identify and complete the examination. This written referral must include the examination to be completed, a diagnosis for the examination (to include CPT and ICD-9 Coding) and the signature of the physician.

Following registration, the patient is directed to the Radiology Registration Desk and Waiting area to allow for additional information verification and entry of the examination orders into the health information system. Patient identification is an important aspect of the services provided by the Radiology Department of Logan Regional Medical Center and the patient's identity will be verified both by records review and the requirement for the patient to verbally indicate their name and date of birth by each person encountered during the examination.

At the completion of the examination, the Radiologic Technologist will review the images produced for quality before allowing the patient to leave. Results of the Radiology examination are forwarded electronically to the patient's physician or health care provider.
